Not an emergency service
This is not an emergency service. Safe Harbor Home Health provides scheduled visits, not round-the-clock supervision or emergency response.
For chest pain, difficulty breathing, a fall with injury, sudden confusion, or any other medical emergency, call 911 first. Nothing on this website should be read as an offer of emergency medical response or as a substitute for calling emergency services.
Requests are not confirmations
Submitting the request form on this site does not begin care. A request becomes a plan of care only after an assessment visit, confirmation of eligibility and benefits, and a physician order where one is required.
Hospital discharges and other time-critical referrals should be made by telephone rather than through the form. Acceptance of a referral is subject to clinical suitability and staffing availability.
Accurate information
You agree that the information you give us is accurate and that you are authorised to provide it, including where you are arranging on behalf of a family member or patient.
Details such as mobility level, equipment, weight, oxygen use, stairs, and access restrictions determine whether we can safely perform the service. If material details are inaccurate or withheld, our crew may have to decline at the door and a charge may still apply.
Fees, insurance, and billing
Prices are quoted before service begins. Coverage by Medicare, Medicaid, a managed-care plan, or a private insurer depends on your specific plan, eligibility, and the reason for the service, and is confirmed by our office rather than by this website.
Any coverage table or fee example shown on this site is a general guide, not a benefits determination or a quote. Where a payer declines, you remain responsible for the balance under the financial responsibility form you sign.
Changes, cancellations, and no-shows
Visits can be rescheduled by calling the intake line. Please give as much notice as you can so the clinician's day can be reallocated.
Where a clinician arrives and cannot safely deliver the visit, no access to the home, the patient is absent, or the environment is unsafe, a missed-visit charge may apply and the physician may be notified.
Acceptable use
You agree not to misuse this site. That includes submitting false requests, attempting to gain unauthorised access, scraping or harvesting content, interfering with the site's operation, or using it for anything unlawful.
We may refuse service, remove content, or restrict access where we reasonably believe these terms have been breached.
Intellectual property
The Safe Harbor names, logos, and lighthouse marks, and the text, images, layout, and code of this site, belong to Safe Harbor Home Health or its licensors. You may view and print pages for your own use or to share with a family member, physician, or case manager.
You may not copy, republish, or adapt the site or its branding for commercial purposes without written permission. Typefaces and photography used on this site are licensed to us and are not sublicensed to you.

This site links to our sister company under common ownership, and may link to hospitals, payers, and public resources. Linking does not mean we endorse or control those sites, and we are not responsible for their content, availability, or practices.
